We are so glad you have decided to take charge of your health! The results of your NAQ will provide profound insight into your current issues as well as identifying where your health could be headed. Talk about an ounce of prevention being worth a pound of cure. 🙂

Please complete these two steps and then expect to receive an email with your invitation to take your personal and individualized NAQ.

Step 1: Provide Consent to have the NAQ emailed to you. Please use the short form below; enter your name and email address.

Step 2: Complete the NAQ. Answer all questions on the NAQ and click “SUBMIT.” The system will let Cathy know when you’re done and she will email you to schedule your follow-up session. The follow-up can be held via phone or in-person.
